Discharge Planning
At Seven Hills we begin discharge planning at admission in order to make sure that the supportive environment necessary for a lasting recovery is in place at the time the patient is discharged. Through ongoing communication with the referring professional, the patient experiences a smooth transition back home with scheduled supportive outpatient counseling. Community resource referrals are provided.
